Family Strengthening Program
– Case Manager

REPORTS DIRECTLY TO: Director of Grant Programs Administration & Compliance

GENERAL FUNCTION: Under the direction of the Director of Grant Programs Administration & Compliance, the Case Manager plays a crucial role in supporting families to build resilience, improve their well-being, and achieve their goals. The Case Manager oversees and coordinates the Family Strengthening Program case management and prevention services through case management best practices, weekly site visits, one-on-one support, observation, file reviews, and tracking of outcomes. To ensure each family’s needs are met, the Case Manager collaborates with Family Strengthening Program staff, youth, and caregivers to develop and implement a service plan that includes the youth’s and caregiver’s strengths and needs, goals for the youth(s) and caregiver(s) to achieve during their participation in the program, and discharge planning to ensure the families are linked to the community resources they need to thrive.

KEY ROLES:

Client Assessment and Intake:

  • Conduct initial intake assessments of families to identify their needs, strengths, and goals.
  • Formulate an appropriate and responsive service plan that addresses identified areas of need.
  • Establish and maintain positive, supportive relationships with youth and caregivers.
  • Visit program sites weekly to check in with the youth and caregivers regarding their program experience, progress towards attaining goals, and additional or changing needs.

Service Coordination:

  • Conduct case coordination in a strengths-based, family-centered, family-driven, and culturally competent manner through a wraparound team-based process in which youth and families participate as partners.
  • Connect families with relevant community resources, such as financial assistance, counseling, childcare, and educational programs.
  • Develop and maintain relationships with community organizations to facilitate access to resources for program participants.
  • Monitor and evaluate the effectiveness of services and adjust as necessary.
  • Assist families and Club staff with the Flex Fund application process.
  • Refer youth and/or families to the BGCBC Life & Resiliency Skills team as needed to help build and practice identified skills as well as to coordinate crisis intervention services.

Case Management:

  • Mindfully integrate a trauma-responsive and person-centered approach into all procedures, practices, and daily interactions.
  • Provide ongoing support and guidance to families throughout their engagement with the program.
  • Conduct regular follow-up meetings to track progress, address challenges, and adjust action plans.
  • Advocate for youth’s and caregivers’ needs and rights within the program and with external agencies. Facilitate communication with external agencies as necessary.

Documentation and Reporting:

  • Responsible for administrative documentation review and approval of program documents, including assessments, service plans, and progress notes.
  • Maintain youth and caregiver confidentiality according to established policies, federal and state statutes, as evidenced by obtaining needed releases of information and maintaining updated releases of information.
  • Prepare and submit regular reports on client progress and program outcomes.
  • Ensure compliance with program policies, procedures, and regulatory requirements.

Qualifications

  • Bachelor’s degree in social work or related Human Services field from an accredited institution is required.
  • Minimum of one year experience working with children in a full-time position.
  • Minimum of one year experience providing strengths-based case management services through a trauma-responsive lens.
  • Knowledge of community resources, service referrals, and linkage.
  • Excellent leadership skills and customer service approach with the ability to effectively communicate with a variety of audiences using maximum discretion when handling sensitive and confidential information.
  • Strong organizational skills with attention to detail and the ability to prioritize multiple tasks.
  • Able to interact with diverse personalities, in a non-emotional, non-confrontational manner always displaying tact, maturity, and flexibility; and the ability to address and de-escalate crisis.
  • CPR and First Aid certification required.
  • Maintain a safe driving record.
  • Proficient with computers and electronic records.
  • Bilingual is a plus (Spanish/English or Creole/English).

Salary: $50,000/year plus benefits
